A 3 year old girl who is small for her age is diagnosed with juvenile rheumatoid arthritis. Her liver and spleen are palpable and her joints are swollen and tender. She is unable to move around freely as before.

The answer is C, Methotrexate.
Aspirin was previously considered the drug of choice for juvenile rheumatoid arthritis (JRA) but not any longer because of its side effects.
First choice are other NSAIDs including selective COX-2 inhibitors.
In case of advanced JRA as in this case or resistant to NSAIDs then DMARDs (Disease Modifying Anti-rheumatic agents) like Methotrexate, Hydroxychloroquine, cyclophosphamide, etc) are given.
Steroids are last choice for JRA and sometimes given along with DMARDs.
