Undoubtedly, intensive care unit is one of the major components of health care system, which provides high quality of care to critically ill patients.being an ICU resident in both government and private sector for nearly 2 years in Khartoum has prompted me to address the obvious shortage of ICU doctors.

recently the medical field has witnessed a gradual increase in the number of ICU ,particularly in private hospitals, which is attributed to the high cost of ICU bed per 24 hours, however, this rise is not met by adequate and selective recruitment of highly trained staff in order to run it the proper way,in other words to improve ICU performance.

the approach of an ICU patient is different, hence, the role of intensivist " a medical specialist in critical care medicine" is essential and can not simply be taken over by a general physician or anaesthetist no matter how brilliant they are.
